As a QA manager and quality coach, I have seen organisations underestimate the strategic value of quality. QA is not a final checkpoint or a technical step. It is risk management, resilience, trust, and the guarantee that what we build works safely and reliably.
The Big Picture is that QA is not just about finding defects. It builds confidence in products, processes, and systems. Every release, automated system, or critical process carries risk. Without robust quality practices, risk translates into financial loss, operational chaos, or harm to people. Strong QA creates predictability, credibility, and the freedom to innovate safely.

NASA’s Mars Climate Orbiter
A mismatch between metric and imperial units in navigation software caused the $125 million spacecraft to fail near Mars.
Lesson: Small misalignments between teams can have massive consequences. Consistent standards and cross-team alignment are essential.
Therac-25 Radiation Overdose
Race conditions and missing safety checks in this medical system caused radiation overdoses during cancer treatment, resulting in at least six deaths and multiple injuries.
Lesson: Safety-critical systems require exhaustive testing, hardware interlocks, and fail-safes. QA in this context saves lives.
